"Welcome Home" is an adult day care center, which provides structured socialization and health monitoring for our clients, as well as respite
for caregivers. We serve seniors in the S.W. Oklahoma City/Moore area who cannot be safely
left alone, who demonstrate a decline of cognitive functioning, or who are just in need of increased
socialization. Our hours of operation are Monday - Friday, 7:00am - 6:00pm.

In addition to a monthly newsletter, we provide a daily activity schedule and special events calendar. The daily activity schedule provides
details about the programs throughout the day. We have time set aside for social hour, reminiscing,
current events, group and individual activities (crafts, games, ect.), health updates and more.
Our day also includes exercise and movement and a nutritious lunch. We go on outings for entertainment
and to participate in the community.
Our monthly newsletter provides information for caregivers regarding upcoming events, points
of interests, health news from the nurse, as well as notes from the social worker, program
director and executive director.

We can serve approximately 36 clients per day. We plan to open a North location soon. With an additional location we
will be able to serve more seniors needs.

At Welcome Home, we ask that our clients attend on a regular basis, or at least 2 days a week to establish continuity. This not only develops
a schedule for your love one, but is also helps them to establish a relationship with the staff
and other clients. With advanced knowledge we can provide for special coverage.

To enroll in our program, an enrollment interview needs to be scheduled in order for the nurse to meet the prospective
client. This meeting takes approximately 45 minutes. This meeting involves an overview of the
program, including our mission, admission criteria, discharge criteria, physician forms and
daily activities. We also conduct a quick assessment to ensure that our services are appropriate
for the prospective client. Before a client begins services with us, we need to have a Physician's
Order form, completed by the primary care physician, testing for Tuberculosis (results included),
and the enrollment paperwork. Information and forms will be provided during the enrollment
interview.

During the enrollment interview, notify the Director about the need for transportation. Transportation is arranged
through Welcome Home. Transportation to the facility will be between 7:00am - 8:00am.
Transportation home will be between 3:00pm - 4:00pm. Transportation is free in the metro area.

Cost are kept as low as possible, as our goal is to allow all families in need of our services to participate.
At this time, full time is $45.00 per day and drop-in rate with lunch is $50.00 per day. Please call and ask for assistance with payment
options, assistance is available for the majority of seniors needing adult day services.
